Reporting to the Manager of Clinic Operations, the Licensed Practical Nurse (LPN) is responsible for providing comprehensive nursing services to patient populations within the Calgary West Central Primary Care Network (CWC PCN).

 

Responsibilities

As a valued team member at the CWC PCN, you will work in conjunction with the physician and/or independently as appropriate to assess the needs of patients, supporting the delivery of high quality, evidence-based health care to the practice population. You will work collaboratively to the full scope of nursing practice as a member of a healthcare team within a family practice environment to enhance the delivery of primary care services to our patients.

Your daily activities will comprise of:

  • Situating patients to consulting rooms, collecting and entering patient vitals in Wolf EMR, and providing health services based on your fully developed nursing knowledge and skills
  • Conducting assessments, initiatives, and implementing management strategies, providing appropriate evaluation, follow-up, and modifications to care plans according to practice protocols; assisting with the delivery of disease management and health promotion, implementing strategies for improving the quality and provision of care
  • Coordinating services with other team members and community resources according to policies, procedures, and established standards
  • Utilizing your exceptional attention to detail, you will be entering statistical data into Wolf EMR for the collection of key performance measures for organizational and provincial reporting
  • Ensuring that the clinic is stocked with medication and general medical supplies; maintaining sterilization protocols, equipment standards and supply levels in clinic exam rooms
  • Being responsible for your own self-directed professional development, participating in CWC PCN education programs and attending multidisciplinary nursing meetings.

Qualifications

The ideal candidate is a Licensed Practical Nurse with a proven track record working with a primary care team to treat and manage patients with health problems efficiently and effectively. You will have exceptional organizational and problem-solving skills and strong communications and customer relations skills.

Education: You are a graduate of an accredited Licensed Practical Nursing Program. Current College of Licensed Practical Nurses of Alberta registration in good standing is required, including injection, medication administration and immunization certification. Phlebotomy experience is considered an asset. Current Health Care Provider CPR is required (BLS Provider from the Heart and Stroke Foundation).

Functional knowledge: You have knowledge of medical equipment and supplies usage according to established standards and procedures and a strong interest in advancing patient-oriented health services via new service delivery models. You are willing to play a leadership role in helping to define these new models. You also have a thorough knowledge of applicable data privacy laws and able to handle confidential and sensitive information.

Experience: You have a minimum of two years’ experience in broad clinical practice required; having clinical experience in a primary care setting is desirable; experience working in high-volume, physician-led clinic environment is preferred. Excellent proficiency with Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint) and EMR systems required.

Background check: A clear Criminal Background check, inclusive of a vulnerable sector search, is required to start. If you have resided outside of Calgary in the past 12 months, you will need to contact the local police or RCMP detachment in your previous residential municipality to be able to obtain a full report.
