Status: Independent Contractor | Vacancies: One | Terms: Project-based work; approximately 20 to 40 hours per month; anticipated end date March 31, 2020 | Hourly rate: $50
Reporting to the Director of Member Services & Engagement and working collaboratively with respective stakeholders, you are responsible for engaging with specific member clinics of the Calgary West Central Primary Care Network (CWC PCN) to assess the current state of PCN resourcing, identify gaps and/or duplication of services, develop resource plans, and obtain consensus on how to implement the new resource plans and effectively manage any required transitions. You will work with clinic management and physician members to come to an agreement on which resources will be provided by various Calgary Zone PCNs and how those resources will be shared.
Current state assessment: Work with the Physician Liaison team to identify clinics where multiple PCNs are supporting the physicians. Meet with physicians and clinics to gain a detailed understanding of PCN services; validate information through discussions and identify gaps, redundancies and duplication of services. Solicit feedback on what is working well and any opportunities for improvement.
Consensus-building: Work with physicians and clinic management to build a membership and resourcing plan for each identified clinic. This may involve the development of a resource-sharing model between two or more PCNs where a memorandum of understanding would be required.
Project Management: Liaise with the Project Management Office team to coordinate project planning and develop a memorandum of understanding where appropriate. Develop implementation and transition plans for clinics and identify stakeholders who will be responsible, accountable, consulted and informed throughout the project. Identify risks and mitigation strategies and use change management tactics in transition plans. Track progress and provide regular updates to the Director of Member Services & Engagement.

You are skilled at facilitating group discussion and building consensus, enabling the group to take control and responsibility for implementing next steps and able to draw out opinions from group members, identifying ways to support the group to move forward.
You are calm under pressure but operate with a sense of urgency. You have excellent listening, written and verbal communication skills, as well as interpersonal and relationship-building skills. Your ability to analyze information, share new and innovative ideas and implement solutions is key.
