Working the full scope of practice, the PCRN with a mental health focus assesses, manages and supports patients as part of an integrated Patient Medical Home (PMH) care team to create and implement effective and relevant quality initiatives. The quality process includes, but is not limited to, developing care plans, evaluating outcomes and providing follow-up with the patient population. The PCRN supports patients by providing comprehensive care, education, and interventions for mental health as well as chronic disease management.
Working with physicians, health professionals, local and regional programming, and regulated by the College and Association of Registered Nurses of Alberta (CARNA), the PCRN is a key player in the creation of healthy patients and satisfied providers.

Chronic disease management
The ideal candidate is a Registered Nurse with a proven track record working within a primary care team. This nurse will have extensive experience in managing patients with mental health conditions and chronic diseases.
Education: Bachelor’s degree in Nursing and current CARNA registration and membership in good standing is required. CNA Psychiatric and Mental Health Nursing Certification required or willingness and eligibility to complete. Canadian Diabetes Educator certification required or willingness and eligibility to complete (minimum 800 hours of practice in diabetes education). Additional certifications in mental health and chronic disease management are desirable. Professional education related to quality improvement (QI) in the workplace and on teams is desirable.
Functional knowledge: Required experience in broad clinical practice, including specific experience in mental health and chronic disease management. Clinical experience in a primary care setting desirable. Previous experience in patient teaching or group facilitation is desirable. Experience with successful QI initiatives in the workplace and on teams is preferred. In the interview, the candidate must be able to provide concrete examples of clinical experience as well as discuss all topics mentioned here to demonstrate knowledge in these areas.
Organization skills: Ability to multi-task and prioritize. Strong organizational skills. Proven ability to work under pressure and meet deadlines in a highly challenging environment.
Communications and customer relations skills: Strong communication, interpersonal, and relationship-building skills; able to handle requests and complaints in a professional, non-confrontational manner. Excellent follow-up and problem-solving skills, with ability to make intuitive decisions. Discretion and ability to handle sensitive information and material appropriately.
Attention to detail: Precision, accuracy, and high attention to detail in all aspects of work.
Team-oriented: Ability to work effectively independently as well as with a team.
Technical: Proficiency in computers, Microsoft Office Suite and Electronic Medical Records systems. Experience in and/or willingness to learn platforms such as Basecamp, Dayforce, and digital health is required.
