Patient Care Coordinators

Patient Care Coordinators (PCCs) help doctors gain a better understanding of their patients' needs. Patient care coordination is a new service that provides member physicians with assistance in gaining a better understanding of patient needs by arranging well-organized visits when patients need to be seen and helping to track achievements in improved patient care.

The PCCs work directly with physician member clinics and availability varies by clinic.

  • Required screening
  • Follow-ups
  • Testing
  • Tracking
