Social Workers

As part of our services to seniors, the CWC PCN social worker assists patients and their families with accessing support and navigating the health system. Referrals are accepted for issues relating to environmental and personal circumstances, with the goal of enhancing living situations and improved quality of life.

The social worker connects with patients as often as required to determine their needs and refer them to appropriate resources for ongoing assistance. Our social worker is based out of the CWC Primary Care Centre at Westbrook and sessions are conducted in the clinic or in the patient’s home.

Social workers assist with a wide range of psycho-social issues such as:

  • Cognitive decline
  • Family dynamics
  • Depression
  • Obesity
  • Need for respite
  • Addictions
  • Grief support
  • Transportation
  • Mobility equipment needs
  • Financial applications/referrals
  • Alternative living environments
  • Requirements for in-home supports
