The Calgary West Central Primary Care Network (CWC PCN) and Alberta Health Services (AHS) are collaborating on an innovative one-year pilot program to provide in-home primary care for vulnerable seniors in 2017 called Seniors Home Based Primary Care (SHBPC).
The SHBPC program is a type of mobile medical home designed to provide home-bound seniors aged 65 and older with a comprehensive care program and team who provide care at home. Up to 24 patients with complex health issues are taking part in the service this year.
The Seniors Home Based Primary Care (SHBPC) interdisciplinary team is guided by a CWC PCN member physicians and includes a geriatric nurse consultant, licensed practical nurse, home care case manager, pharmacist and social worker supported by a medical office assistant.
This program helps specific patients to “age-in-place” with respect, dignity and comfort while reducing emergency room visits and hospitalizations.
