Patient Care Teams (PCTs) work with your family doctor and consist of mental health professionals and nurses.
The objective of PCTs is to increase the emphasis on chronic disease management, health promotion, disease and injury prevention and improved care of medically complex and chronically ill patients. If your family physician is a member of the CWC PCN, they may work with these professionals to provide enhanced patient care. Availability varies by clinic.
You can see your family doctor and their care team members from the comfort of your own home. Talk to your clinician about your preferred format for appointments.
The Calgary West Central PCN is made up of 465 physicians in over 135 clinics.find one near you
