Patient Care Teams

Patient Care Teams (PCTs) work with your family doctor and consist of Behavioural Health Consultants and Nurses.

The objective of PCTs is to increase the emphasis on chronic disease management, health promotion, disease and injury prevention and improved care of medically complex and chronically ill patients. If your family physician is a member of the CWC PCN, they may work with these professionals to provide enhanced patient care. Availability varies by clinic.

Many physicians have access to Patient Care Team members, like nurses and Behavioural Health Consultants who can both provide support for mental health.

As the COVID-19 pandemic continues to change the way we work and live, many people are experiencing increased stress levels and unforeseen obstacles that impact mood and resilience.

Mental Health Assist is a single-session virtual care program for anyone* that has increased stress and trouble coping.

At a single sixty-minute session – by phone or video – you will have the undivided attention of a mental health professional who will listen and provide support, education, coaching and resources.

  • Sessions are one per patient, per concern
  • Virtual sessions are done through the secure CWC PCN Patient Connect app.
