Are you looking for a doctor/medical home for you or your family? Some of our CWC PCN member physicians are accepting new patients. There are several ways for you to find a medical home:
The Primary Care Centre (PCC) located near Chinook Centre acts as a temporary medical home or transition clinic for patients currently seeking a permanent family physician. It is an appointment based clinic that does not see patients on a long-term basis. Visiting unattached patients are directed to the Patient Web Registry and provided with a list of physicians in our service area who are currently accepting patients. Physicians who work in the PCC are members of the CWC PCN. The PCC is open weekdays from 9 a.m. – 9 p.m. and 1 - 4 p.m. on weekends and holidays. For a referral to the PCC, please contact Health Link at 811.
In partnership with other Calgary area Primary Care Networks, the CWC PCN offers a web registry where patients sign up to be matched with a family physician in their geographic area. To register, visit www.albertafindadoctor.ca or call Health Link at 811 for a referral to the Primary Care Centre. This registry is for patients who currently do not have a family physician. Please note: this process may take some time depending on availability and patient requirements.
