Social Workers

As part of our services, the CWC PCN social work team is available to support all age ranges — seniors, adults, children and youth and their families — with accessing support and navigating the health system. Referrals are accepted for issues relating to environmental and personal circumstances, with the goal of enhancing living situations and improved quality of life.

The Social Worker connects with patients as often as required to determine their needs and refer them to appropriate resources for ongoing assistance. Our Social Workers are based out of the Primary Care Centre and sessions are conducted in the clinic, by phone or video, or in the patient’s home.

Social Workers assist with a wide range of psycho-social issues such as:

  • Cognitive decline
  • Family dynamics
  • Family supports
  • Family violence
  • Mental health and addiction supports
  • Need for respite
  • Addictions
  • Grief support
  • Transportation
  • Mobility equipment needs
  • Financial applications/referrals
  • Housing supports
  • Requirements for in-home supports
  • Basic needs (food, medication, clothing, furniture, etc.)
